I've loved the look of the Kao-Tuo since starting as a DM. They look so doofy. But, it wasn't until I read about them that I truly found them to be great threats.
They seem like Gibbering Mouthers, except with more communication and hyperfixations. I'd expect them to spout out nonsense about an overlord for an entire month, create it as a god, worship it for a few months, then start blabbiling about another false god and just cycling that way forever.
I also think "Time for Crab" is funny.
The kidnapping part came from a question in my head that was like "How can I make them evil? Truly show they have no compassion for anyone?" and then I added unwilling sacrifice on top and that seemed like a pretty decent answer. Think about if you were the child in this scenario; scared, lonely, worried. Not sure if you were going to see your family again. These strange fish people talking in a language you don't understand, barking on and on about somethings and nothings. So, the critical part is saving the child, not killing the cult, although that is an option.
